Here's how it works:

The patient is given your practice’s customized intake digital form. It is a paper form with an underlying micro-dot pattern. Each form has a unique micro-dot pattern specific to that patient’s form.
The patient uses EaseMD’s Digital Pen to write and fill-in the form just as he or she would with any other form.
While writing the digital pen’s optical reader is capturing the coded information from the underlying micro-dot pattern.
The patient brings back the form and pen to the reception desk.
