Bubble Sheets for Patient Intake

Here's how it works:

  1. The patient is given a pre-printed (or printed on demand) bubble sheet by the receptionist.
  2. The patient fills in the form with any dark ink pen or pencil.
  3. The patient brings back the form to the reception desk.
  4. The receptionist scans the form.
  5. On-screen software allows the receptionist to correct any reading errors.
  6. With a click of a button, the patient information is sent into the patient’s progress note for that day’s visit.
