Bubble Sheets for Patient Intake
Yes or no? You and your nurse spend too much time documenting the subjective elements of a patient’s progress note. If the answer is "yes" you can easily save time by
having the patient complete an EaseMD Systems bubble sheet, which works just like a familiar, standardized test form. Scanning the form at the reception desk automatically loads
the patient information into his or her progress note — ready for you to review before you meet with the patient.
Key Benefits:
- Bubble Sheets are familiar forms, resembling standardized test forms, making them easy and simple for patients to use.
- Any pen or pencil can be used to fill out the form.
- The subjective portion of your progress note is filled in automatically and accurately — before you see the patient.
- A complete copy (electronic image) is automatically retained for legal and audit use.
- Bubble Sheets represent minimal additional cost cost — only the time needed to create a customized form to fit the needs of your office.
- Bubble Sheets can be several pages. You can have different forms for different types of patients (new patients, established patients, diabetic patients, post-op appointments, etc).
- Bubble Sheets can be produced in different languages.
Here's how it works:
- The patient is given a pre-printed (or printed on demand) bubble sheet by the receptionist.
- The patient fills in the form with any dark ink pen or pencil.
- The patient brings back the form to the reception desk.
- The receptionist scans the form.
- On-screen software allows the receptionist to correct any reading errors.
- With a click of a button, the patient information is sent into the patient’s progress note for that day’s visit.
